This information form is required of all our new patients. For your convenience, you may print the form, complete it and bring it with you to your initial office visit. This will help to minimize wait time. As always, all personal information is kept strictly confidential and protected according to HIPPA regulations and our office Privacy Policy.

If you are a new patient the following list will help your remember what items and information to bring with you to Brandon Orthopedic

1. Insurance cards and claims filing information.
2. Medical information (xrays, records, driver license or state id, summaries of past illnesses or hospitalizations) if request by one of our staff.
3. List of current medications and dosages.
4. Also, please be sure to follow any special instruction, such as fasting, that you may have received before your visit.
